Step 2: We’ll Call You
We’ll review your information, ask a few quick questions, and answer anything you’d like to know. Then, our care coordinators will help you schedule your consultation.
Step 3: Come to Your First Visit
Arrive 10 minutes early for a brief welcome meeting before your appointment. We’re looking forward to meeting you!
For Providers: How to Refer a Patient for SPRAVATO®
Step 1:
Complete the Provider Referral Form and gather copies (front & back) of the patient’s pharmacy and medical insurance cards.
Step 2:
Upload the completed referral form and documents through our secure portal.
Step 3:
Our team will take it from here and follow up as needed.
